The Transaxillary Approach

THE BREAST AUGMENTATION PROCEDURE

Breast augmentation involves making a small incision to insert a breast implant into the breast area in order to enlarge the breast. The surgery is commonly performed on an outpatient basis at a hospital or state-of-the-art surgical unit while the patient is under a general anesthesia and asleep. There are several possible locations for the small incision that will be used for inserting the breast implant. The most frequent technique utilized by Dr. Shaw involves an incision made in the axilla or armpit. Another technique involves making an incision in the breast crease. A third technique makes an incision around the areola (the darker skin surrounding the nipple). The implant is usually placed below the chest muscle and sometimes over the muscle depending upon a number of factors including breast shape and size, patient lifestyle, as well as both the patient and surgeon’s preference. The best technique will be determined at your consultation appointment. Be sure to discuss your goals and expectations.

During surgery, the breast tissue is raised to create an open pocket under the breast tissue or more commonly, beneath the chest wall muscle. Inserting an implant behind each breast can increase a woman’s breast size by one or more bra cup sizes. Implants may contain new cohesive silicone gel or saline solution (similar to saltwater). Surgery typically lasts from 1 1/2 hours.

AFTER SURGERY

Your breast may be swollen and sore for a week or two. Bruising is usually absent. You will begin massage exercises on your 1 week post op visit when the sutures are removed. We believe that by maintaining a large pocket surrounding your breast implants you can lower the risk of capsular contraction. These exercises will become part of your daily routine. Vigorous exercise routines may resume 4 – 6 weeks following your surgery. You may return to work in 5 to 7 days as long as you are off of pain medications.

RECOVERY PROCESS

Generally, post-operative instructions call for plenty of rest and limited movement in order to speed up the healing process and recovery time. A sports bra and upper elastic binder is applied right after surgery to aid the healing process and to minimize movement of the breasts. Massage techniques and suture removal will occur at one week after surgery. The sports bra will need to be worn for several weeks.

Patients do report some discomfort associated with subpectoral breast implant surgery. This pain can be effectively treated with oral medication. While complications are rare, patients can minimize potential problems by carefully following the directions given by the physician after surgery.
